Please fill out forms 1-3  before coming to counseling.  Form #4 Consent to release information, is used only as needed.

Please bring a photocopy of your insurance card and your co-pay. I am able to except either cash or check.

Please remember to cancel your appointment with 24 hours notice whenever possible. Thank you.
